BEFORE BEGINNING TREATMENT
The following are merely suggestions and are not intended to provide a legal opinion as to procedures. The suggestions come from experiences of cancer travelers; however, the procedures may require consultation with an attorney specializing in a specific area.
If you need to be admitted to a hospital for any type of testing or treatment, the hospital may ask if you have two forms:
- A Living Will (addresses whether you want to stay alive using artificial means).
- A Designation of Surrogate (enables you to appoint someone to make medical decisions on your behalf in the event you are unable to so).
It is also advisable to designate someone to handle medical insurance claims and other matters with your insurance company if someone other than your spouse will be handling the claims. Additionally, you should give medical consent for the treatment of your children if you do not have a spouse.
Sample forms follow for the sole purpose of giving you an idea of what is included in such forms. Many times the hospital or insurance company will have its own form to complete.
